3.JW202表格电子版和复印件1张（提供扫描件备用，提醒本人将原表格扫描留存，学习期间每年使用一次）；
4.住宿费、学费、保险费（1年有效期）缴费发票；
5.指定医院体检报告原件及复印件。

三、老生申请居留许可延期的有关要求。

Thosewhocontinuetostudyforamaster'sdegreeinHEBUST,theofficeofHEBUSTwillissueanewJW202forminadvancetoconfirmtheResidencePermitextension.
Thosewhoapplyforamaster'sdegreeinotherschool,mustleavethecampusbeforetheResidencePermitexpirationdate,andthesubsequentvisaproceduresareassistedbythewill-benewschool.(friendlyadvice:pleasecontacttheResidencePermitinadvancetoapplyforavisa.)
老生申请居留许可延期时，提前30天提交以下文件。
